The Hepatitis B and C Coordinator will provide essential support for patients and physicians, helping manage and coordinate care for individuals affected by Hepatitis B and C. The coordinator will assist with patient care, liaise with healthcare professionals, and streamline access to diagnostic tests and treatments, ensuring a smooth patient journey. Candidates who speak Vietnamese or Chinese are strongly preferred to help better serve diverse patient communities.

Key Responsibilities:

• Hepatitis B Coordination:
• Collaborate with physicians and patients to facilitate timely access to Hepatitis B medications.
• Assist with the reimbursement process for Hepatitis B treatments, ensuring patients have access to necessary funding.
• Monitor treatment initiation and provide ongoing patient support to ensure continuity of care.
• Organize and coordinate lab work, including blood tests, fibroscans, and ultrasound bookings.
• Maintain up-to-date records of patient treatment progress and regularly communicate with the physician to provide updates.
• Hepatitis C Coordination:
• Work with physicians and community organizations to facilitate Hepatitis C testing and diagnosis.
• Organize and streamline the process of Hepatitis C treatment initiation and follow-up.
• Help coordinate care between healthcare teams and provide patient education and support throughout their treatment journey.
• Assist in locating lost-to-follow-up patients and ensuring they re-enter care.
• Support the management of patient data, ensuring it is accurate, confidential, and accessible to the to the physician and healthcare team.

This position offers a unique opportunity to work closely with a dedicated physician and provide vital care cooination to improve patient outcomes for individuals affected by Hepatitis B and C, particularly in Vietnamese or Chinese speaking communities.

Qualifications:

• Strong organizational skills with the ability to manage multiple tasks efficiently.
• Excellent communication and interpersonal skills to effectively collaborate with physicians, patients, and healthcare teams.
• Vietnamese or other Chinese language skills would be a strong asset to help serve a diverse patient population.
• Experience working in healthcare coordination or patient support is an asset.
• Knowledge of Hepatitis B and C treatments and care pathways is an advantage.
• Phlebotomy skills are a valuable asset but not mandatory for the role. systems is preferred.

Preferred Education and Experience:

• A diploma in healthcare administration, or a related field.
• Experience working in hepatology, infectious diseases, or a similar area of healthcare is an advantage.
• Experience in working with diverse communities and understanding their healthcare needs.
