CARE QUALITY LEADER

Reporting to the Resident Service Manager, the Care Quality Leader (CQL) provides frontline leadership and supervision to the RNs, LPNs, and RCAs working in the assigned unit. The CQL oversees care delivery, clinical performance, and resource utilization (staffing for assigned cluster of 2 to 4 neighbourhoods of 50-65 residents). They coordinate with allied health professionals and support services to build positive partnership with residents and families to achieve the care goal of each resident.

As a frontline leader, the CQL works with the leadership team to carry out care enhancement projects, such as accreditation, eMAR, and PointofCare implementation. The CQL is responsible for fostering an open and positive work environment for continuous learning and quality improvement.

DUTIES AND RESPONSIBILITIES

1. Provides clinical supervision and leadership to direct report to exceed standard care for residents and to consistently achieve person-centered care goals by reviewing 24-hour shift reports to address significant changes in a resident’s condition and high-risk clinical occurrences, such as use of restraints, unexplained injuries, falls, wounds, behavioral episodes, medication errors and near misses, and infection prevention and control.

2. Coordinates daily routines, assignments, and staffing levels. Adjusts work methods to maximize effectiveness and efficiency.

3. Provides orientation to new staff and facilitates a smooth onboarding process. Evaluates performance through probationary reviews, annual performance appraisals, and on-the-job evaluations.

4. Provides continuous guidance, coaching, and supervision to all assigned staff to assess and support ongoing professional development and assurance of care skills. Follows through with staff to meet mandatory training requirements.

5. Monitors and oversees the use of care resources by managing supplies, inventory, and staffing.

6. Actively participates in the implementation of various clinical initiatives to continuously improve the quality of care in the ever-changing health care environment.

7. Screens admission packages, make recommendations, and coordinates with the interdisciplinary team for a smooth move-in experience for new residents.

8. Conducts regular and unannounced audits for quality assurance.

9. Work collaboratively with Care Quality Leaders of other neighborhood clusters to form and strong team and to provide coverage for one another.

10. Coordinates with the interdisciplinary care professionals, physicians, and support services to problem solve and promote the best care options and best quality possible for residents according to their goal.

11. Monitors and follows through with the completion of RAI-MDS assessment, care plan development and reevaluation, and the update of the ADL sheet and bedside care plan. Audits the accuracy of assessment data and provides monthly care delivery reviews to the Resident Service Manager.

12. Schedules and presents in-care conferences on behalf of the primary nurse if needed for the assigned unit and complete the care conference notes.

13. Responds to resident and family concerns and complaints. Addresses, resolves, or escalates the issue in an effective and timely manner.

14. Maintains a safe environment for residents and staff. Conducts incident investigation for assigned neighborhoods and completes incident reports. Coordinates with facility services for regular audits and inspections of care equipment. Reports repair and replacement needs.

15. Maintains a current, broad knowledge of local community and regional initiatives that may impact resident care, resource access, or practice guidelines.

QUALIFICATIONS

Education & Experience:

  • Minimum Bachelor’s in Nursing; post-graduate courses and/or certification in gerontology an asset
  • Five (5) years recent experience related care experience in a long-term care including demonstrated competencies in leadership or the equivalent combination of education, training, and experience
  • Current practicing registration as a Registered Nurse with BCCNM

  • Demonstrated ability to adapt to the changing health system and an understanding the broad determinants of health
  • Demonstrated ability to develop and evaluate health education resources
  • Demonstrated ability to critically read and use current health research in evidence-based practice
  • Comprehensive knowledge of other health care disciplines and their role in resident care
  • Demonstrated ability to identify community resources appropriate to residents and resident population needs
  • Demonstrated ability to effectively use a computerized client care information system and standard MS Office suite applications
  • Ability to speak and write Chinese an asset
