Opportunity

We are currently recruiting for the role of Senior’s Community Connector to join our Senior’s team in Surrey, BC. The Seniors Community Connector works closely with senior clients, and the North Surrey Division of Family Practice (FP) to respond to the client’s identified needs. The connector participates in the development of a wellness plan and warmly refers and connect clients into appropriate services including specialized seniors services at DIVERSEcity, external supports such as the City of Surrey recreation and senior’s services, other settlement and community organizations, and senior serving organizations. This role is part of the Social Prescribing model, which aims to increase social support, physical activity and good nutrition for seniors are who are risk of frailty.

Key Responsibilities:

Oversees the day-to-day operation of the program by ensuring that the necessary facilities and equipment are in place, program guidelines and policies are adhered to, and program standards and licensing requirements are met;
Builds referral relationships with Fraser Health and the Division of FP;
Receives referrals of clients who have been received a “social prescription” of the program through the Division of FP;
Provides outreach to frail, socially isolated and/or disengaged older adults in Surrey and North Delta communities. Responds to enquiries, via telephone, email, text or needs assessment;
Under the direction of the manager, conducts initial client assessment and develops clients’ optimized wellness plan that links them to appropriate community seniors programs and services that are acceptable and suitable to the senior;
Provides information about the systems of services, outlines possible options and makes appropriate referrals to community resources;
Initiates and performs follow ups, advocacy and assisted referrals;
Participates in the development of a personalised plan based on wellness goals of seniors;
Supports seniors in accessing services that will help them achieve that plan;
Supports clients in tracking their progress to achieve their plan;
Initiates and performs follow ups, advocacy and assisted referrals;
Ensures that all program participants feel supported, empowered and are re-engaging socially and physically in our community;
Fosters partnerships and establishes networks with other service providers serving the targeted population group in the community based seniors sector;
Stays current on formal and informal community based seniors programs and services available in the community;
Liaises with other sites and community partners to create opportunities for community involvement and identify and respond to gaps in community based seniors services;
Represents the Social Prescribing Program at meetings, conferences and community events;
Participates in the development of program policies, procedures, manuals, training materials, evaluation and measurement tools;
Organizes information sessions on topics that are of interest to the client group.
Qualified Candidates will possess:

Diploma in a related human/social service related field;
Two years recent related program/project coordination experience working with seniors; or an equivalent combination of education, training & experience;
Experience working with newcomer seniors issues and knowledge of the settlement sector;
Demonstrated experience and ability working with diverse populations;
Excellent written and spoken English;
Verbal and written fluency in either Mandarin, Punjabi or Korean;
Excellent organizational and time management skills;
Excellent knowledge of community resources, services and supports in the community based seniors sector and ability to network and build partnerships;
Ability to work with minimal supervision and meet deadlines;
Ability to use a computer, including MS Office Suite and Zoom or MS Teams;
Access to a vehicle and valid drivers license.
This position requires union membership. A Criminal Record Check is a requirement of employment.
